Boundary Area
Anyone asking to register with the practice must live within our practice area. Our list is open.
When anyone asks to register, reception staff will check your address against the map of our area.
If you live within the area, you will be asked to fill out form GMS1, which will need to be fully completed so that we can obtain your medical records from your previous doctor (if you have one).

Catchment Area
NHS Frauds team has advised us that we should ask for ID upon registration. Failure to provide ID will not preclude registration.

- Passport
- Birth Certificate
- Marriage Certificate
- Local Authority Rent Card
- Bank/Building Society cards/Statements
- Payslip
- Letter from Benefits Agency/Benefit Book/Signing on Card
- Letter from Benefits Agency/Benefit Book/Signing on Card
- Library Card
- Health Club Card
- Private Rent Book
Non-urgent advice: Please Note
All new patients who register with the practice will be offered the opportunity to participate in a new patient health check, these are run by a nurse or our GP Assistant.
